You apparently must beat ALL ABCD rankers to begin challenging regional rankers and than world rankers. From what I've heard challenge is not required.
You must beat every ranker for each class, meaning, 14 for rankers for each class .i.e. D,C,B,A only after all 56 class rankers are defeated will you be able to face regional rankers
Yes, you have to beat the rankers D - A in every location you don't need to beat any challenges. After that I believe they challenge you via talking to you in Hard-Wired.
